Packing Procedure

1. Label the specimen tube(s) with the patient's name, social security number and blood draw date.
2. Place specimen tube(s) into styrofoam container and securely seal the container with the red tape provided.
3. Place styrofoam container into plastic bag and seal the bag.
4. Place the sealed bag, containing the styrofoam container, and accompanying paperwork into the provided cardboard mailer.
5. Place cardboard mailer into a FedEx diagnostic specimen envelope.
6. Attach the pre-addressed airbill and send to:

Paperwork

1. Provide patient information, referring physician information, diagnosis, and test to be performed using a Millenova Immunology Laboratory requisition form for each patient.
2. Physician accounts are to be set up prior to sending in specimen. Please contact Dolly Ayala at (312) 274-1928 for assistance.
3. If charges are the patient's responsibility, please enclose payment by check or credit card. No samples will be processed unless payment is received with the sample.

Final Checklist

Please be careful to

	Label each specimen tube with the patient's name, social security number, and blood draw date.
	Fill out a separate requisition form for each patient.
	Enclose all paperwork in the mailer.
	Clearly indicate the billing information.
	If charges are the patient's responsibility, then payment must be enclosed.
